This card is made with a piece of dsp measuring 12 x 5 1/2.  Score at 4" and 8".  Then cut at angle on the far right hand section.  Apply adhesive ONLY to the lower left  and bottom edge of the third panel.  Do NOT add adhesive to the top edge or it will limit the piece you insert to the pocket. (That's what I mistakenly did.  :(
